Bowling Options:

Lane Sponsorship — $250 (includes your name or logo on our entryway sponsor board)
Reserve a lane for up to 6 bowlers — $180
Individual bowlers — $30

Domino’s Pizza ($15 each – choice of cheese, sausage, or pepperoni), along with pretzels, candy, and soda, will be available for purchase! Please fill out your information below to reserve your spot and be part of this meaningful event. After the form is submitted, you will be directed to the Donate page to complete your payment. Add your total and enter this amount in the "Other" box on the donate page. Payment can be made by Paypal, Credit/Debit card, Venmo or Check. Please add a note to your payment indicating whether it is for a sponsorship, bowling lane(s), or specific bowler(s).
